As part of our responsibility to keep young people safe and to meet UK Scouts requirements, all accidents, incidents, and concerns must be recorded and, where appropriate, reported correctly. This process ensures we have clear records, can support young people effectively, and meet our safeguarding and safety obligations.

Please follow the process below when anything occurs during sessions or activities:

1. Logging on OSM

Please go to Members and select:

  • Accident Form – for any injury or medical issue
  • Incident Log – for behaviour, safeguarding, or external incidents

2. Safeguarding

If there is any safeguarding concern, this must be reported to the UK Scouts Safeguarding Team within 24 hours. Speak to the GLV as soon as possible for support and next steps. Please remember:

  • Record factual information only
  • Do not investigate yourself
  • If there is immediate danger, call 999 first

3. Medical Incidents / Near Misses

Any medical incidents or near misses should be discussed with the GLV, as these may need to be reported to The Scout Association in line with reporting requirements.

4. Behaviour Incidents (Yellow/Red Cards)

If a Yellow or Red card is issued:

  • Complete the Incident Log on OSM.
  • Go to the young person’s Member Profile → Additional Information
  • Add Behaviour Incident Logged, including:
    • Date
    • Card type (Yellow/Red)
    • Reference to the related OSM incident log

5. Informing Parents/Carers

Parents or carers should be informed of any accident or incident.
Keep communication clear, calm, and factual.

6. Good Practice

  • Follow the Yellow Card (Code of Conduct) at all times
  • Record accurate, factual details
  • Focus on the behaviour, not the young person
  • Ensure all concerns are properly recorded and, where required, reported

In short: log it on OSM, inform parents, speak to the GLV where needed, update the member record if applicable, and report safeguarding concerns within 24 hours.
